New Patients
During your first visit, we will ask for important background information.
Please take the time to fill out the patient forms
to save time on your first visit.
We ask you to arrive fifteen minutes before your appointment to ensure your
first visit goes smoothly and to allow time for any paper work or questions you
may have.
If you are using dental insurance, please bring your dental insurance card. Be
careful not to confuse it with your medical insurance card, as they are not
the same.

Our payment options include cash, check, debit card, Visa, Mastercard, American Express, and Discover.
We also offer up to sixty months of financing. With good credit, we offer up to 12 months interest-free financing.
